FC Nova Youth Programs - Uniform Party Form

This form is to be used by parents that volunteered to be Uniform Coordinators to indicate that they will be attending the Uniform Party. At the Uniform Party, we will be organizing the Youth League and Tots Program uniforms into to individual and team bundles. Approximately 20 volunteers are needed. The purpose of this page is to ensure that we have enough volunteers to do the job, and to make sure that we don't have too many individuals show up.

When we have enough volunteers, it will be indicated on this page.
